Adult Championship awards order form
- Coach or team manager please submit this form no later than 2 weeks from the end of the session; late orders will not be accepted.
- Orders submitted by anyone other than your coach/team manager will not be accepted.
- A member of the Sports City team will contact you when your order is ready to pick up. Orders generally take 2-3 weeks to arrive.
- Orders not picked up within six weeks of notification will be forfeited.
- Seasonal hoodie and crewneck colors change every 3 months, no exceptions will be made.
- Seasonal options are available in adult sizes only, unless the color is also available on the Youth Championship Award Form.
