Session Champion Award Order

Coach or team manager please submit this form no later than 2 weeks from the end of the session; late orders will not be accepted. Orders submitted by anyone other than your coach/team manager will not be accepted. A member of the Sports City team will contact you when your order is ready to pick up. Orders generally take 2-3 weeks to arrive. Orders not picked up within six weeks of notification will be forfeited.

If you are placing an order for upgraded items, an invoice will be attached to managers account and suspended for the balance due. This must be paid before the release of the order.

T-shirt or Sweatshirt Details

If ordering T-shirts please list the quantity for each size. Number of shirts can be no greater than number of players on roster.

Size options are Youth Small to Adult 2X
